Case management support & stabilization of the employment relationship

Prolonged periods of illness are often accompanied by uncertainty as to whether one can continue performing one's current job for health reasons. This situation is equally unsatisfactory for both employees and employers.

Our case management (CM) specifically addresses this point of uncertainty or disorientation. We mediate between all parties involved to maintain or fully restore the ability of those affected to work. In a close coordination process between the sick employee, the company, and the rehabilitation provider, we examine whether and how the employee and the company can be reunited.

Often, simply adapting the workplace with appropriate tools is sufficient. In many other cases, the individual's valuable experience and skills can be retained within the company through internal transfer and made available elsewhere.

Together, we identify new and innovative solution strategies where necessary and initiate the necessary processes. To this end, we establish contact with other institutions, such as the advisory bodies of the integration offices or the employment agency, and provide support in locating suitable jobs.

Target group

This offer is aimed at employees receiving sick pay. Participation requires approval from the responsible rehabilitation funding agency.

The aim of the offer is the sustainable integration of employees with health impairments into companies in order to avoid unemployment.

Duration & Course

Once commissioned by the pension or health insurance provider, or another rehabilitation funding provider, you can start at any time. The maximum duration of support for each individual is six months. If no reintegration into the labor market has occurred after this period, we will prepare a personalized final report with recommendations for future integration activities.

Financial support

Funding is provided by the responsible rehabilitation funding agency.
